Comparsion of C.I.S.H. with TAH







Abstract
Pelviscopic hysterectomy has been introduced as a surgical alternative to the standard methods of abdominal hysterectomy.
C.LS.H.(Classic Intrafascial S.E.M.M. Hysterectomy) is an intrafascial cylindriform enucleation of the cervix and supracervical amputation of the uterus, leaving intact the highly vascularized extrafascial fibrous cervical tissue. With C.LS.H technique pelvic support could be maintained and transvaginal sexual sensation may be less likely to be impaired because of the preservation of the cardinal and uterosacral ligaments.
To evaluate this new surgical procedure, a case control study of the operating time, blood loss, pain medication, complication, and hospital stay was conducted, comparing C. LS. H. with total abdominal hysterectomy(TAH). Fifty patients undergoing C.I.S.H. from March 1993 to February 1995 were compared with 50 patients undergoing TAH during the same period.
The average operating time for C.I.S.H. was 155.38¡¾ 5.86 minutes whereas for TAH it was 71.62¡¾ 3.69 minutes(p<0.05). The average length of hospital stay for C.LS.H. was 3.06¡¾ 0.18 days shorter than THA(p<0.05). During hospitalization the C.I.S.H. group required an average of 31.2¡¾ 2.6mg of nalbuphine while the TAH group required 55.2¡¾ 3.2mg of nalbuphine(p<0. 05).
We conclnded that C.I.S.H. can be performed as safe as TAH, and have advantages such as a early recovery and shorter hospital stay.
